Qualification Checking
The following details are required for any institutions from
which the applicant has received a qualification.
- Name of institution
- Address of institution including postcode
- Phone number of institution (if known)
- Qualifications obtained with dates.
Professional Membership Checking
The following details are required for any professional body
of which the applicant is a member.
- Name of professional body
- Address including postcode
- Phone number (if known)
- Membership number (if known).
Employment Reference Checking
The following details are required for any employment held
by the applicant during the previous 10 years.
- Company name
- Contact at company
- Address including postcode
- Phone number (if known)
- Position held with dates.
Personal Reference
Details of two personal referees are required.
- Name
- Address including postcode
- Phone number (if known)
- Nature of relationship.
