Pre-Employment Health Assessment
Information Required
To complete a Pre-Employment Health Check, the candidate must provide
the following details:
Personal Details
Information such as date of birth, next of kin and name and address
of GP must be provided.
Immunisation Status
The candidate must indicate whether or not they have ever suffered
from any of a list of illnesses or whether or not they have been
vaccinated against these illnesses. Contact details for the doctors
or organisations holding the candidate's immunisation records must
be provided.
Occupational and Medical History
The candidate must provide information on their occupational and
medical history including whether or not they have ever been a hospital
patient, suffered from any of a list of illnesses, been absent from
work in the previous 12 months, or are currently on any medication.
The candidate must also provide information on whether or not they
smoke or consume alcohol.
Family History
The following information about the candidate's parents, brothers
and sisters should be provided:
- Age (if living)
- If deceased
- Age at death
- Cause of Death
- State of health
