Night Worker Medical Assessment

Information Required

To complete a Night Worker Medical Assessment the employee will need to indicate if they have ever suffered from any of a list of illnesses or medical conditions and if they smoke or consume alcohol. They will also be asked if they have ever received hospital treatment, how recently they last saw their own GP and if they are currently taking any medication.

Required information on the employee's occupational history includes the amount of sick days they took during the previous year, whether or not they have experienced health problems associated with work or whether or not they have ever been denied a job on health grounds.
