Night Worker Medical Follow-Up

Information Required

To complete a Night Worker Medical Follow-Up the employee will need to indicate if they have ever suffered from any of a list of illnesses or medical conditions and if they smoke or consume alcohol. They will also be asked if they have ever received hospital treatment, how recently they last saw their own GP and if they are currently taking any medication.

Required information on the employee's occupational history includes the length of time for which they have been a night worker, the amount of sick days they took during the previous year and whether or not they have experienced health problems associated with work.
